Fees For Services
Fees for services will be charged to the credit card kept on file. We will request the credit card authorization form be completed during the intake process. While Dr. Mitchell is in-network with a small amount of insurance panels, not all of our clinicians are. If you have out-of- network benefits, we will be happy to provide you with a superbill for you to submit for reimbursement.
If you do not show up for a scheduled appointment, or do not cancel within 48 hours, the full fee will be charged to the credit card on file.
